If you are not acquainted with another dentist, your insurance company’s provider directory or its member support line may be able to assist you. Further, all patients can click here to find an ADA Member Dentist near you to provide continuous and emergency care.
If you would like to request your records or transfer your records to your new provider, please complete the Records Request Form and e-mail back to records@brighttiger.com.
